If the server's running Windows Server 2012 R2, you don't need to install the provider. The provider is located with the setup files in \amd64\Setup\msi\iSCSITargetSMISProvider.msi, and on the VMM server in \Program Files\Microsoft System Center 2012\Virtual Machine Manager\Setup\Msi\iSCSITargetProv\iSCSITargetSMISProvider.msi.
